Dependent Audit 2023

Beginning in mid-June, Trinity Health will be conducting a dependent audit to confirm that only eligible individuals are covered under our benefit plans. This effort is part of our continuous review of programs to ensure we are being good stewards of our resources. We have engaged Alight Solutions to validate dependent status on our behalf, for any family members currently covered under a Trinity Health medical, dental or vision plan.

Colleagues will be required to provide proof of dependent relationship for each family member covered (e.g., marriage certificate, tax documentation, birth certificate, etc.). Dependents that cannot be verified will be removed from plan coverage effective Sept. 30, 2023, prior to open enrollment.

IMPORTANT DEADLINE: AUGUST 18, 2023

How do I submit documentation? 

You have three options to submit documentation:

  1. Online:  Upload documents on the Alight member portal

    2. You will receive login and password information in the initial letter from Alight the week of 6/20
  2. Secure Fax: 1-877-965-9555
  3. Mail: Dependent Verification Center, PO Box 1414, Lincolnshire, IL 60069-1414
