Our Investments Improve the Health of Our Communities
Trinity Health's Community Health Institute annually commits grant funding support to community-based organizations to accelerate community health improvements. Grant funding may only be requested by a Trinity Health hospital; we do not make grants externally. If you're interested in seeking grant funding, a strong partnership with your local Trinity Health hospital Community Health & Well-Being team is essential.
Grants include:
Transforming Communities Initiative (TCI) is an innovative funding and technical assistance initiative advancing policy, system, and environmental change coupled with addressing social needs to make measurable progress toward health equity for populations who are poor and vulnerable in Trinity Health communities. TCI advances community partnerships and accelerates community health improvement by implementing evidence-based strategies that directly impact the priority needs as identified by the hospital's community health needs assessment (CHNA).
