PACE
Trinity Health Programs of All-Inclusive Care (PACE) provides compassionate, comprehensive care for each participant using a care circle approach. This model of care, called the interdisciplinary team, treats the whole person with a full-circle view.
Home-Based Care
Our clinicians have demonstrated a special passion for healing the mind, body, and spirit of every patient we treat. Even those confined to home. Our skilled nurses and therapists will partner with our patients, their physicians, and caregivers to create a personalized care plan for this phase of their healthcare journey. Our home-based care offerings include companion care, remote patient monitoring, and hospice.

Health and Wellness Care
Beyond traditional health services, we also provide health and wellness care including independent retail location Urgent Care Centers, Specialty Pharmacies providing specialized medicines for chronic care or other debilitating conditions, and an Outreach Lab where we're able to send and receive lab work.
Specialty Pharmacy
When it comes to specialty medications for complex, chronic, and rare diseases, our highly-trained pharmacists and specialized care team take great measures to provide guidance on drug interactions, allergies, and therapy management to each patient as well as provide customized education, follow-up, and side effect management.
Patients of our Specialty Pharmacy receive easy access to medications (including fast, free shipping), continuous clinical evaluation, ongoing health monitoring, assessment of educational needs, and management of medication use. They are also automatically enrolled in a therapy-specific patient management program ensuring optimal care.

Diabetes Prevention Program
Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Approximately 96 million American adults—more than 1 in 3—have prediabetes. Of those with prediabetes, more than 80% don’t know they have it. There are ways to reduce your risk and prevent diabetes.

Our Military and Veterans Health Care Program is committed to providing convenient access to high-quality health services to meet the specific needs of military service members, veterans, and their family members. Many of our doctors and staff have completed dedicated training to help understand and respond to the health concerns and challenges specific to veterans and many of the military communities we serve. MediGold
MediGold is a hospital-sponsored Medicare Advantage organization that provides high-quality, cost-effective health care at an affordable price. MediGold serves seniors and other Medicare beneficiaries. As a not-for-profit MA organization founded by Ohio doctors and hospitals, we offer exceptional benefits for hospital care, doctor visits, prescription drugs and more. Service areas being select counties in Ohio, Idaho, Iowa, and in 2022 New York.
Global Health Volunteers
Since 1989, Global Health Volunteers has sent teams of health care professionals to provincial villages and distressed urban areas in Latin America and the Caribbean to help people with little or no access to professional health care. Founded as Global Health Ministry, today's Global Health Volunteers has served thousands of people through the dedicated work of hundreds of volunteers.
